Mental Health Group Facilitator

Mental Health Group Facilitator / Social Worker

Reports to: Program Director / Designee

FLSA: Full-Time Non-Exempt Summary of Essential Functions

This position provides support to the Mental Health & Wellness program both onsite and online. Onsite the Program Facilitator presents the 32 Modules that comprise the MH&W program to assembled clients using streaming videos along with additional specific and interactive activities. Online the Program Facilitator may interact with online clients in addition to tracking and compiling data related to client participation and completion of each Module. For specific clients, this position may facilitate group therapy.

Duties and Responsibilities

  • Registers clients who meet the criteria for IOP (Intensive Outpatient Program), PHP (Partial Hospitalization Program), and private pay or insurances.in person or via tele or zoom
  • Calls and schedules clients as requested, prepares and completes intake packet, assembles new client binder, and works with LCSW/LMFT with new client assessments
  • Welcomes clients to the appropriate classroom and begins the program on time, following the daily schedule carefully to manage time elements and provide for regular client breaks
  • Is familiar with the Mental Health & Wellness Program and able to discuss content questions within scope of practice and classification
  • Leads all activities that comprise the program including appropriate games that stimulate brain function and group therapy discussions within scope of practice and licensure or certification
  • Completes required forms including billing, notes for patient’s chart, and reports for RNs, LCSWs, LMFTs, and psychiatrists
  • Leads group therapy discussions within the scope of practice and licensure or certification.
  • Maintains confidentiality of all client information as required by law 9. Follows all pertinent regulations such as a non-smoking campus
  • Monitors client performance to identify potential learning-related challenges 11. Complies with all ongoing educational requirements in a timely manner including CPR certification, Safety education, HIPAA, ADA, and etc.
  • Attends events sponsored by Cognitive Care or specific community events as requested and/or assist with selected marketing activities
  • Must be able to travel to and from presentations or camp meetings or health clinics or general conferences with occasional overnight travel.

Responsible for:

  • Keeping classroom clean and disinfected
  • Ensuring all materials are in class and ready for clients to use such as quiz printouts, pencils, activities, and sign-in sheets
  • Organizing client charts (e.g., chart audits, closing or discharge documents, linkage notes)
  • Tracking of client reassessments, ensuring they are completed on time, and scheduling with the LCSW or LMFT
  • Additional duties as required.

Qualifications

  • Able to speak, read, write, understand, and communicate effectively in English.
  • Bachelor’s Degree preferred.
  • Has two years of prior experience in group therapy / group facilitation preferred.
  • Competent and proficient in using job-related equipment, computer, copy machine, et cetera.
  • Must be trustworthy, honest, accurate, and detail oriented.
  • Able to exhibit high emotional intelligence skills.
  • Able to deal pleasantly and courteously with all employees, contractors, consultants, administration, guests, physicians, clients, and the general public, exhibiting high Emotional Intelligence skills.
  • Able to reason and think through the work process and solve problems in a timely manner.
  • Able to work independently without constant supervision.
  • Understands own knowledge limits and asks for help in answering questions as needed in order to ensure accuracy.

Job Description

The HR Administrative Assistant provides support to all department managers and staff by handling a variety of interactions between the organization. Manage all employee documents, payroll, time and attendance for over 10+ employees.

Duties and Responsibilities

  • Process onboarding for all employees, contractors and consultants.
  • Capturing employee data and maintaining employee records
  • Collecting work records and timesheets
  • Preparing payroll data

Mental Health & Wellness Marketer

Full Job Description

This position provides administrative marketing support to the development and implementation of marketing goals, targets, and strategies for the brand. 

Duties and responsibilities

  • Collects and maintains a marketing database, creating and interpreting a variety of reports related to market research and helping to identify marketing trends. 
  • Engages in data gathering and evaluation to ensure clients and customers are satisfied by the service provided. 
  • Assists with scheduling marketing meetings and campaigns, communicates and coordinates with operations, social media, and branding regarding confirmation of event dates, and serves as media liaison.
  • Promotes services, determines customer’s needs and requirements and makes recommendations to customers of the various solutions the organization can offer.
  • Assures brand management is appropriately executed.
  • Build strong relationships with clients and the community.
  • Plans and develops marketing, community relations and community education activities.
  • Follows plan development, supervises implementation of marketing, community relations and community education activities and campaigns.

Summary of Essential Functions

The Licensed Clinical Social Worker provides high-quality patient care while displaying adequate skills in assessments, psychodiagnosis, treatment planning, group therapy, individual therapy, family therapy, and discharge planning. This position provides support to the Mental Health & Wellness program both onsite and online.

Duties and Responsibilities

  • Provide individual counseling to patients, and family counseling when needed.
  • Conduct weekly groups.
  • Assist the Mental Health Workers with interviewing families, support persons, and patients to gather pertinent information for assessment completion.
  • Assist the Mental Health Workers with referring patients to community services and/or additional health care professionals as needed.
  • Assist Mental Health Workers by acting as liaison between PHECC and Psychiatrist as needed.
  • Manage the care of patients during their attendance of the program and during the discharge process.
  • Reports any significant changes in condition to Psychiatrist and Administrator (as needed).
  • Analyzes patient data and prioritizes patient needs accordingly.
  • Performs all aspects of patient care in an environment that optimizes patient safety while reducing the likelihood of medical/medication/health care errors.
  • Collaborates with patients, Mental Health Workers, and all additional parties involved with patient’s care to develop an individualized patient care plan.
  • Collaborates with Mental Health Workers to update and/or revise care plans as patient needs and/or condition has changed.
  • Performs crisis intervention counseling in necessary
  • As a patient advocate, ensuring the patient’s psychosocial needs are met.
  • Maintain patient confidentiality as required per HIPAA guidelines.
  • Additional duties as required.
